A Note from our Manager of Foster Care and Adoption:
Thank you for your interest in the Focus on Youth Foster Care Program. Focus on Youth is a private agency specializing in therapeutic foster care and special needs adoptions. We are a faith-based agency, grounded in Christian values and principles. We provide our foster parents with superior training, 24-hour support services from our licensed social worker staff, reimbursement for the care of each youth, and a family friendly environment.
Please review ODJFS 5101:2-5-31, the state rule regarding the transfer of a foster home. This rule includes all the criteria necessary to complete a transfer. The first step is to complete the release of information forms below which will allow us to obtain required records from your current licensing agency and begin the transfer request. Once this information is received, the records will be reviewed to determine whether a transfer is appropriate.
