Before Your First Visit
Please fill out and submit the forms below prior to your first visit. If you have not yet contacted us, please contact us to check for availability before completing the forms.
Your First Visit
- A licensed clinician will be matched with the youth/family’s needs and conduct a comprehensive assessment to determine which services will best meet those needs.
- The clinician collaborates with families to develop individual goals for the youth/family. The goals are reviewed on a regular basis.
- Clinicians regularly engage with the parents and/or guardian regarding treatment progress and additional needs because parent involvement provides the best outcome.
- Sessions with the clinician are held weekly or as determined by the treatment team members.
- Sessions take place at Focus on Youth or other locations for service can be arranged when available.
- Crisis support is available if the need is determined.
