Alternative Care and Respite Application

If you have made the decision to become an approved alternative care or respite provider, below you will find requirements and the application. Alternative Care and/or Respite services can be provided in your own home or a foster caregiver’s home.

Please note the following:

1. All applicants and adult household members (AHM) need to sign paperwork where indicated.
2. All applicants and adult household members need to get fingerprints and background checks
3. The approval process for alternative care or respite is an annual process. You will receive a letter in the mail requesting updated information
4. Respite providers are required to attend 12 hours of training at Focus on Youth. Click here to view our preplacement training calendar.
