Congratulations on turning in your application! You’ve taken a very important step towards your goal of becoming a certified foster / foster-to-adopt family. Your next step is to complete the paperwork listed below. Remember to contact your family assessor if you have any questions or concerns. They’re here to support you.
Child Characteristics Checklist
Employee Verification Form (fill out top half and assessor will fax to employer)
Juvenile Release of Information (if applicable, see Juvenile Release of Information tab to the left.)
Landlord Notice (if applicable)
Medical Addendums for each household member
Copy of Current Car Insurance Policy (please continue to provide an updated copy when you renew)
Divorce Decree
Copy of Marriage License
Financial Documents (Documents should have your name and address):
- Last year tax statement (1040)
- Two months proof of most recent income (must be dated within 6 months of licensure) (ALL incomes listed on the financial statement)
- Two months proof of most recent rent/mortgage payment (must be dated within 6 months of licensure)
- Two months proof of most recent gas/electric bill payment (must be dated within 6 months of licensure)
- Two months proof of most recent phone bill payment (must be dated within 6 months of licensure)
- Two months proof of most recent water bill payment (must be dated within 6 months of licensure) (If home has well water – complete well water check)
Proof of your residency for the past five years for each adult household member (please see the residency validation form for acceptable documentation)
Immunization Records for all household members, included Pertussis and annual Influenza vaccinations
Copy of Daycare License (if you intend to use a daycare for foster children)
Copy of Vaccinations for Animals (if applicable)
County Local Crime Check for each adult household member
Local Crime Check for each adult household member
BCI & FBI Fingerprints (Call Focus on Youth to make an appointment) for each adult household member. The cost for this is $50/person and they expire after one year.
Medical Statements and Instructions (Keep in mind that Medical Statements expire 1 year from the date signed by your medical professional.)
Create Evacuation Plans
– One for EACH FLOOR of your home (including Basement)
– Must include Primary and Second Exit Routes for each floor
– Indicate where your Tornado Shelter is located
– Hang a copy on each applicable floor and provide copies to assessor
Create First Aid Kit
Emergency Numbers List (hang up in home)
Carbon monoxide detectors for EACH FLOOR of your home (including Basement) and near all sleeping areas of the home
ALL medications must be locked (prescription & over the counter)
Review Safety Audit and make any necessary changes
If you have a swimming pool, please review this form to ensure you comply with state guidelines.
Any weapons or firearms need to be stored in a locked manner, with ammunition stored in a separated locked area.
Alcoholic beverages will be stored in a safe manner which prevents a child’s access as developmentally appropriate.
All applicants will need to take all 24 hours of Pre-Placement Training and also take CPR & First Aid. Please note, Pre-Placement classes expire 18 months from the date you took the class.
